Registering Coaches, and Volunteers

1. Fill out a HYSA Volunteer Disclosure  & Membership Form
2. Attach a copy of  your drivers license (or other government ID with date of birth and signature)
3. Attach a 1”x 1” photo of yourself (head shot without hat or sunglasses)
4. Send $5 (one-year pass) (last names beginning with M through Z)
$10 (two-year pass) (last names beginning with A through L)
Checks made payable to “HYSA”
5. Include a self addressed stamped envelope for returning pass
6. Mail to:

Registering  Players and/or  Teams

1. Fill out Team Reservation Form
2. Attach a check made out to BI-HYSA for the team fee
3. Mail to:   
        BI-HYSA
        Attn:  Registrar

        Kailua-Kona, Hi 96740
4. Submit your  “ Team Packet” which consist of the following items:
   • completed US Youth Soccer – Team Roster (Please list all Coaches and Team Adminstrators on your Team Roster)
   • a copy(you keep the originals to be used as your medical consent) of  the completed Membership Form for each player and Coach(Coach retains the original and must have it in their possession for all team events)
   • a 1”x 1” head shot photo of each player attached to their membership form
   • a copy of each players legal Birth Certificate attached to their membership form if playing HYSA for the first time.
   • Mail this team packet along with a check covering all the membership fees for each player to:
     (Please note that one check is preferred for membership fees covering all players on a team)
